Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate care management activities for members in Florida.
Key Responsibilities
Evaluate member needs and recommend care plans to address barriers
to accessing appropriate care
Develop and monitor ongoing care plans while collaborating with
providers and community resources
Provide education to members and their families on healthcare
options and facilitate access to necessary services
Required Qualifications
Degree from an accredited school of nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in nursing or care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Must reside in Florida for the remote position
Ability to work Monday - Friday, 8AM - 5PM EST
